VIM has a wide range of programs available to serve your medical needs.
PRIMARY MEDICAL CARE Primary Medical Care is provided at no-charge for the treatment and diagnosis of acute and chronic conditions/illnesses that are non-emergent but do require medical attention, including routine preventive health screenings and general care coordination.
SPECIALTY MEDICAL CARE Limited Specialty Medical Care is provided at no-charge by referral to specialists such as Cardiology, Gynecology, Ophthalmology/Optometry, Podiatry, Dermatology and Neurology. These services may be provided in the clinic or in private practices in the community. Limited dental services may also be available. The types of specialty care services changes regularly and all services may not always be available.
LABRATORY TESTING, X-RAYS, ULTRASOUNDS, MRIs, CT SCANS Laboratory Testing, X-Rays, Ultrasounds, MRIs, CT scans are all provided at no-charge when necessary for diagnosis and treatment, including routine laboratory testing.
CARE MANAGEMENT Care management is provided to facilitate communications between a patient and medical provider, with pharmacies and all referred services.
DIABETES CARE MANAGEMENT Diabetes Care Management is available by referral for non-compliant and high-risk patients who will benefit from a caring professional to learn about their diagnosis and receive the in-person and telephone support needed to make nutritional and healthy choices to manage their diabetes.
HEALTH EDUCATION Health Education is available on several topics including Living With Diabetes and Hypertension: What Do I Need To Know, both of which are offered year-round. Other classes may be offered at different times.
SOCIAL SERVICE ASSESSMENT & COMMUNITY REFERRALS Social Service Assessment and Community Referrals are available to all patients to try to help alleviate needs affecting a person's overall physical health. Common referrals include those for shelter and housing, food security, transportation, domestic violence and legal resources to name just a few.
HOMELESS MEDICAL OUTREACH Homeless Medical Outreach is provided by teams of volunteers who "go-to-the-streets" every Tuesday evening to provide medical care and support to some of our Valley's unhoused residents, primarily in the east Valley. Teams are always led by a physician or nurse practitioner and include other volunteer nurses, medical assistants, scribes and translators. Additionally, we partner with Jewish Family Services of San Diego to provide medical and nursing services to unhoused residents in the west Valley.
VISION CLINICS, DENTAL CLINICS, COVID & FLU VACCINATION CLINICS At various times throughout the year, special clinics and services are arranged for patients and the public, such as Vision Clinics, Dental Clinics, COVID and Flu Vaccination Clinics.
